Medicine are pleased to be a provider in several health care plans.
Medicare -
As a participating provider with Medicare, we welcome
Medicare patients and are pleased to submit your Medicare and
secondary insurance claim forms on your behalf. You will be responsible
for all co-pays, deductibles, and services which are not covered
by Medicare.
We do not participate in any Medicare HMO products.
Therefore, Medicare beneficiaries who elect to join an HMO as
an alternative to traditional Medicare will be responsible for
payment of our charges at the time of service.
Medicaid -
If you are covered by "traditional" Medicaid, we must
have a current copy of your insurance card in order to treat you.
We do not participate in any Medicaid MCO products (AmeriGroup,
PeachState, or WellCare). If you participate in one of these plans,
our services will not be covered by Medicaid. Therefore, you will
be responsible for payment at the time of service.
Workers Compensation -
If you are injured through your employment, we will file
your services to your workers' compensation carrier.
All patients with workers' compensation cases must have
authorization from their employer or workers' compensation carrier.
Without proper authorization, you will be expected to pay for
your entire balance. You must provide the required information
to our office at your initial appointment to assist us on the
processing of your claim.
Managed Care -
To accommodate the diversity of approaches to health
insurance in today's managed care environment, we participate
with a large number of managed care plans.
Many managed care plans require that you first obtain
a referral or authorization from your primary care physician (pcp)
before seeing us. Services which are rendered
without prior authorization from your pcp will not reimbursed
by your insurance plan. Therefore, if your managed care plan requires
a referral, you must bring in the referral with you to your appointment.
If you do not provide a referral, you will be responsible for
payment of your bill. Services which are denied due to a lack
of referral are the financial responsibility of the patient.
All co-pays and deductibles required by your insurance
plan are expected to be paid at the time of service.
