Office Visits

We will initiate the registration process by telephone. Therefore, the patient should have a copy of his or her insurance card available when calling.

Please arrive 10 minutes before your scheduled appointment time in order to complete the necessary registration forms. If your visit is covered by insurance, we will need to make a copy of your insurance card(s). If your managed care plan requires a referral or authorization, you must provide a referral from your primary care physician.

We respect your time and make every effort to see each patient promptly. However, because unforeseeable problems can develop, unavoidable delays can occur. We sincerely appreciate your cooperation at these times.

After your examination, your physician may give you prescriptions, instructions, or other recommendations. We encourage you to ask any questions at that time. At the conclusion of your visit, the physician will give you a medical report which summarizes the physician's treatment plan, any applicable work restrictions, and information regarding your next appointment.
