  • Payment Policies

  • We contract with many insurance plans as a convenience to our patients. Please provide us with your complete insurance information.
  • If we participate with your insurance plan, we will file an insurance claim to your insurance carrier.
  • We expect our patients to pay for all co-pays and deductibles, as specified by your insurance benefits contract, at the time of service. For your convenience, payment may be made by cash, personal checks or major credit card.
  • If you do not have insurance, you will be responsible for payment of our services at the time of service. If financial hardship exists which will prevent prompt payment of our bill, we will consider payment arrangements. This matter should be discussed with our patient account specialist.
  • Regardless of your insurance coverage, please understand that you, the patient, are ultimately responsible for payment. When we render services, we charge the services to you, the patient, not your insurance company. We consider payment of fees to be your direct financial responsibility.
  • We allow 45 days for your insurance carrier to pay. If services are not paid within that time, we will bill you for any balance remaining for the services.
